Referrals

ENQUIRIES AND REFERRALS

 

The referral process starts with a detailed conversation about services required and what LIFE101 has to offer.  From this conversation we will make a decision, in principle, as to whether or not we will accept the referral.

Following this decision we will send out a referral form for completion.  We expect this form to be fully completed as well as an indication of the reporting method required. An immediate risk assessment is then made by LIFE101.

Once we have received the referral form and the referral has been accepted, we may require a contractual meeting.

Contractual Meetings

This meeting is an opportunity for the referral person/case manager or lead profession to present the case, outlining the social history and needs of the family.  Discussion will focus on the ways in which LIFE101 facilitate the referral and address particular needs of the child.(ren) young person(s) parent(s), referring agency This meeting also allows LIFE101 to ensure that timescales are addressed and, processes, such as:

  • Allocated worker(s).

  • Individual needs borne in mind e.g. religious observances and dietary and linguistic needs.

  • If there are any other significant person(s) involved in the case that need to be kept informed and the process of reporting.
